Homecare Holding is the leading provider of home healthcare in the Czech Republic, with regional operations in Prague and Pilsen. The company provides post-operative care, monitoring patients’ conditions and administers medications in a home environment. It focuses on highly specialized procedures such as peritoneal dialysis, parenteral nutrition wound care, physiotherapy and care for palliative patients. With around 50 nurses, Homecare Holding provides care to over a thousand patients annually.

Stratum invested in Homecare Holding in 2020, supporting the company’s transformation and digitalization, introducing new services and specializations and integrating one add-on acquisition. In March 2024, Stratum successfully divested its entire stake to Penta Hospitals CZ, marking the fund’s first exit.
